Job Title: Resident Care Coordinator - Interim
Reports To: Director of Nursing Services (DNS)
Compensation: $25 - $30 / hour
FLSA Status: Full-Time/Non-Exempt

As the Resident Care Coordinator, you will not only be working as a part of a cohesive and interdisciplinary resident care team, but you will also be supporting nursing services in their ongoing endeavor to deliver comprehensive, holistic health models. You will be supported by caring and competent individuals that are passionate about the Macdonald Residence community.
Job Summary: The Resident Care Coordinator (RCC) is responsible for assuring that medical services provided to residents are coordinated and supported. The RCC is a connection point between direct care staff, nursing, life enrichment staff, community-based providers and other staff if needed. This position identifies and participates in the implementation of systemic improvements and assists with training and support for direct care staff. This position may be required to cover direct care shifts if needed. This position is a part of the team that manages scheduling.
Essential Functions and Responsibilities
On-call and Direct Care Responsibilities
· Acts as on-call staff and fills in day shifts when requested by the Director of Nursing
· Assists residents with activities of daily living, medication, treatments, and other care while encouraging self-care and independence
· Monitors and assists Lead Med Aide and Direct Care Staff to ensure all medications have been re-ordered and residents do not run out of medications
· Observes changes in resident's status, needs, or preferences, promptly reporting all changes to the nurse on site or on call
· Assists residents with ADL’s as needed
· Adheres to emergency protocols, including performing CPR and administering first aid as necessary
Administrative Responsibilities
· Participates in creating Day and Swing shift Direct Care/Nursing schedule, schedules must be posted by the 15th of the previous month
· Willing to be on-call for scheduling one – two weeks per month
· Attendance tracking
· Works in collaboration with Admissions team
· Assists nursing with screening and evaluations for potential admission
· Monthly vital sheets: Assign vitals to Direct Care by the 1st of each month
· Creates login for new employees/agency for Point Click Care. Will need to be a “super User”
· Reorder medical supplies through McKesson
· Reorder incontinent supplies through Olson Medical Services once a month
· Maintains inventory and reorders applesauce, juice, and ensures in collaboration with Nutritional Services
· Sit in on Direct Care interviews
· Assists with quarterly Physician’s Orders
· Maintains/updates Acuity Roster
· Assist with maintaining pending faxes
· Maintains refrigerator temperatures in the med room
· Assists with creating initial Service Plan for new admissions
· Email weekly report for 213 to Case Manager
· Attends monthly meetings with Summit Team
· Assists with writing TSP’s
· Work with RN to maintain Alert Board
· Work with DNS to create Direct Care agenda for Direct Care meetings and reserves room for meeting
· Documents pertinent information in residents' charts and elsewhere per policy (ie..Incident Reports)
· Reads staff communication records and maintains familiarity with resident records as required
· Other Responsibilities
· Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for residents
· Demonstrates knowledge of and follows infection control procedures
· Adheres to home health and hospice protocols
· Adheres to all established policies and procedures
Supervisory Responsibilities: No providing oversight of residents’, their needs and safety, but no direct supervision of staff.

Education and/or Experience:
· Must have high school diploma or general education degree
· Must have a minimum of five years related experience and/or training related to Direct Care
· Two or more years of experience providing training and support
· Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Language Skills:
· Ability to read, and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
· Ability to write routine reports and correspondence.
· Ability to effectively communicate, understand and follow written and oral instructions.
Reasoning Ability:
· Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
· Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registrations:
· First Aid
· Mandated trainings by ODHS
Work Environment:
The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
